Taking lutein: fat matters, the time of day barely

The short answer: lutein is absorbed through fat digestion β which is why the meal decides, not the clock. In the most precise comparative study on the subject, the lutein value in blood rose by 207 per cent at the same dose when the meal contained 36 grams of fat, and by only 88 per cent with 3 grams. Vitamin E and beta-carotene from the same investigation were not disturbed by the fat amount. And anyone watching the clock is watching the wrong variable: the highest serum value after a single dose is reached only around 16 hours later.
That already describes the practice β with the main meal, not on an empty stomach. The rest of this article explains where the numbers come from, why the same tablet holds substances with quite different demands, and from when anything can be measured at all.
Why fat is decisive
Lutein is a xanthophyll and therefore fat-soluble. It does not leave the gut as a free molecule but built into micelles, which form only with bile salts and fat. Without fat in the gut the means of transport is missing β the substance travels on instead of being absorbed.
How large that effect is was measured by a Dutch research group in a clean experimental design. Four groups of 14 to 15 volunteers each received, over two seven-day periods, one of four preparations daily with a low-fat warm meal: vitamin E (50 mg), a mixture of alpha- and beta-carotene (8 mg), lutein esters (corresponding to 8 mg of lutein) or placebo. The difference lay solely in the bread spread served alongside β low-fat with 3 grams or high-fat with 36 grams of fat (Am J Clin Nutr 2000;71(5):1187β1193, PMID 10799382).

Only one row of this table responds to the fat, and it happens to be the one with the lutein. For vitamin E and the two carotenes the differences between low-fat and high-fat were not statistically confirmed; the authors concluded that these substances manage with a limited amount of fat, while lutein esters need more. The likely reason lies in the chemistry: the ester must first be separated from its fatty acids in the gut by fat-splitting enzymes, and those enzymes work where fat is being digested.
How much fat β and what happens between 3 and 36 grams
The honest answer at this point is: nobody knows precisely. The study compared two points; it did not measure a curve. Where between 3 and 36 grams the threshold lies beyond which absorption no longer improves is open β it could be at 10 grams or at 25.
In practice this means: an ordinary warm meal with oil, butter, cheese, egg, fish or meat, not the apple in between. 36 grams of fat sounds like a lot, but it is the order of magnitude of a lunch with pan-fried fish or a breakfast with scrambled eggs and cheese. The same applies to lutein from food: a tablespoon of oil over the salad is, with vegetables, the single most effective step a plate knows.

The time of day: what is measurable
For the question "morning or evening" there is no solid evidence with lutein β simply because the substance eludes the question. That is shown by an investigation in which eight adults received a single dose of lutein and/or beta-carotene dissolved in oil and then gave thirteen blood samples over 840 hours. The mean serum value for lutein reached its maximum after 16 hours; beta-carotene had a first peak after 6 and a second after 32 hours (Am J Clin Nutr 1995;62(3):604β610, PMID 7661123).
A substance whose peak lies half a day after intake and which circulates in the blood for weeks has no sensible time of day. What remains is regularity: the level results from the sum of the past weeks, not from this morning's tablet. Anyone who hangs the intake on the meal they never skip anyway makes the only decision that counts here.
The same investigation supplies a second practical point in passing. When lutein and beta-carotene were given at the same dose, the area under the lutein curve fell to 54 to 61 per cent of the value lutein reached on its own. In the other direction the effect was inconsistent: lutein lowered the beta-carotene value in five people and raised it in three. Carotenoids therefore compete for the same absorption route β anyone additionally taking a pure beta-carotene product should not take it with the same meal.
What else is in the same tablet
This is where the table above becomes practical. Eye supplements are almost always combinations, and the substances they contain place different demands on the meal. Our Vitamin A + E + lutein contains 10 mg of lutein from Tagetes erecta extract per tablet, 400 Β΅g of vitamin A (50 % NRV) and 20 mg of vitamin E (167 % NRV). According to the Dutch study the vitamin E would cope with 3 grams of fat as well β the lutein would not. The meal therefore follows the most demanding component, and in this pack that is the lutein.
The approved claim for this combination likewise does not hang on the lutein: Vitamin A contributes to the maintenance of normal vision. For vitamin E the claim is Vitamin E contributes to the protection of cells from oxidative stress β a statement about cells in general, not about the eye. For lutein and zeaxanthin there is no approved claim; which four nutrients may carry the vision sentence at all is set out in Eye nutrients: which claims are approved.
Other fat-soluble products may go with the same meal β Vitamin D3 with K2, for instance, uses the same fat and does not compete with the carotenoids for the absorption route. One meal serves both.
Whether the pack says "lutein" or "lutein esters" changes nothing about the timing, but a great deal about the amount: 20 mg of esters are around 11 mg of lutein. In a comparative study the ester form was even better bioavailable than free lutein (J Nutr 2002;132(12):3668β3673, PMID 12468605).
How long until something changes

The second widespread misconception concerns not the timing but the time scale. Anyone who takes lutein and notices nothing after a week has done nothing wrong β the measurable changes run in weeks and months.

The four weeks come from a European multicentre study with 400 volunteers aged 25 to 45. At 15 mg of lutein daily from tagetes extract the serum value for lutein rose roughly fivefold and that for zeaxanthin roughly twofold, even though no zeaxanthin was supplied at all; in the Spanish participants the plateau was reached after four weeks. No noteworthy adverse effects occurred β with one exception worth knowing: carotenodermia, a harmless yellowish discolouration of the skin (Clin Sci 2002;102(4):447β456, PMID 11914107).
The macular pigment reacts more slowly still. In an investigation with two people who took lutein esters corresponding to 30 mg of free lutein for 140 days, the optical density of the macular pigment only began to rise 20 to 40 days after the start, then steadily by 1.13 milli-absorbance units per day. After stopping, the serum value fell back to the starting value within 40 to 50 days, while the pigment density stayed at its level (Exp Eye Res 1997;65(1):57β62, PMID 9237865). Two people are not a body of evidence, and a measured pigment density says nothing about a benefit for seeing β it only shows the time frames in which this substance moves at all.
What slows absorption
Anything that disturbs fat digestion also disturbs the absorption of lutein. Two cases are documented and practically relevant.
Orlistat. The active substance inhibits fat splitting in the gut. The European product information for Xenical notes that the absorption of the fat-soluble vitamins A, D, E and K may be impaired; a multivitamin product should therefore be taken at least two hours after orlistat or at bedtime (EMA, Xenical product information). The same logic applies to lutein, even though it is not named there explicitly. Anyone taking such a medicine settles the intervals with their doctor or pharmacist.
Fat replacers. In two placebo-controlled studies the non-absorbable fat replacer sucrose polyester clearly lowered carotenoid values in the blood: 12.4 grams daily reduced beta-carotene by 34 and lycopene by 52 per cent, with smaller but likewise significant declines for lutein, zeaxanthin and vitamin E (Am J Clin Nutr 1995;62(3):591β597, PMID 7661121). In the EU such ingredients hardly play a role β the finding is remarkable nonetheless, because it shows that it has to be absorbable fat and not merely a fat-like taste.

Frequently asked questions
Should I take lutein on an empty stomach? No β with this substance that is the worst option. Without fat in the gut the means of transport for absorption is missing, and the difference, at 88 versus 207 per cent plasma rise, is greater than any difference between two products. The rule "take on an empty stomach" comes from water-soluble substances and does not transfer.
Morning or evening? With lutein that is secondary. The serum peak lies around 16 hours after intake, and the level builds over weeks. What matters is that it is the meal with the most fat β for most people lunch or dinner β and that it happens regularly.
Can I take lutein together with other supplements? With other fat-soluble nutrients such as vitamin D3 or K2 there is nothing against it, quite the opposite: they need the same fat-containing meal. Caution only with an additional pure beta-carotene product β given at the same dose, beta-carotene reduced lutein absorption to a good half. Such products belong with a different meal.
What if I forget a day? Nothing worth mentioning. The serum value reaches a plateau after about four weeks of daily intake and needs 40 to 50 days after stopping to fall back to the starting level. A forgotten tablet carries no weight on that scale β and a double dose the next day accordingly achieves nothing either.
